Welcome to our practice! We are very excited that you have chosen us for your dental care. We realize you have options in choosing your dental provider and we appreciate your trust and confidence in our practice!

We would like to provide you with some important information about our practice, your first visit, and our policies. Please take some time to read it thoroughly as it will answer many questions you may have.

What To Expect for A New Patient Visit:

At your first appointment, our pediatric dentist, Dr. Mitko will complete a comprehensive oral examination. This visit will be approximately 45 minutes for children. This includes a complete review of medical and dental history, nutrition/oral hygiene counseling, review of any oral habits, all necessary x-rays, extra/intraoral photos (if necessary), oral cancer screening, TMJ evaluation, dental development assessment, examination of the teeth and soft tissues. Your child will also receive a cleaning and fluoride treatment. Don’t worry, if your child is under age 2, our entire staff is highly trained in behavior management and these initial visits are all about sensory exploration (exposing children to all the different sights, sounds, feelings and tastes at a dental visit) as well as anticipatory guidance for parents/caregivers ( information on how to best set your child up for great oral health). We may not get to everything at the first visit, but we will work our way towards it as cooperation increases over time.

For our younger patients, we ask that only one parent come into the room after any x-rays are taken, as our rooms are small and too many spectators can be distracting. We also ask that you not bring other siblings in the treatment room, as we want to make sure we focus 100% of our attention on your child being seen at the moment. Other children in the room can be a distraction for the provider and the patient. Please keep this in mind when scheduling appointments for multiple children. Some parents opt to make appointments on separate dates if they have several young children, none of whom can be in a room independently.  If a younger sibling is nervous and wants to peek in at an older sibling’s successful visit, please let our staff know and we can arrange for it. We are committed to making dental visits fun and informative for everyone!

After the visit, Dr. Mitko will discuss any findings with you, develop a treatment plan that you are comfortable with, and then your child will be scheduled for treatment, if needed.

Paperwork:

Please be prepared for your initial appointment by completing the new patient paperwork ahead of time. You can find the paperwork here:https://www.restorationsmilesdental.com/patient-forms/

We ask that you email the forms to hello@restorationsmilesdental.com prior to your dental appointment. If you plan to bring the completed forms with you to your appointment, please arrive 15 minutes prior to your appointment to allow us to input your information.

Records:

Please contact your previous dental office and have them send over any x-rays for your child  in the last 2 years. Also, please have them send over any pertinent treatment notes (ie. if there is a complicated dental treatment history). If we do not have x-rays less than 6 months old prior to the first visit, we will plan to take a new set of x-rays.

Financial/Insurance:

If you plan to use dental insurance, please email us (hello@restorationsmilesdental.com) a copy of your card (front/back) prior to your appointment so we may verify coverage. It is your responsibility to know if your insurance is in or out-of-network with our providers.

Payment is due at the time of service. We also have an in-office membership and financing options available and will be happy to discuss all options with you.

Directions/Parking:

Patient parking is located in the large parking lot to the right side of the building (NOT IN GARAGE). Use the walkway on the right side to the front entrance. Take the elevator up to the 2nd floor and make a left. If you prefer to take the stairs, they are located past the elevator on the right. Only handicapped parking is available in the garage at the assigned handicapped spots.

Confirmations:

We will reach out to confirm this appointment 1 week prior, AND 48 hours prior to your appointment via email, text and/or phone call.  Please let us know that you are coming by confirming the appointment. If we do not receive a response confirmation 48 business hours prior to your scheduled visit, we may reschedule your appointment due to high demand. Monday appointments need to be confirmed Friday before 2:00pm.

Cancellations/Missed Appointment:

We ask that you make every effort to keep your appointments.  It is important to understand that when forgetting or canceling appointments at the last minute, small businesses lose the opportunity to fill that appointment time, and patients miss out on receiving care. For the benefit and respect of all our patients and professionals, all appointments are subject to the following terms:

  • We require at least 48 hours notice for changes. If you cancel within 48 hours for ANY reason or do not show for your appointment, a $50 fee will be applied to your account.
  • Monday appointments need to be confirmed by Friday 2:00pm.
  • After 3 late cancellations/missed appointments we reserve the right to dismiss you from our practice, or only offer same day appointments.
  • We allow a 15 minute grace period after your scheduled appointment. If you arrive more than 15 minutes past your scheduled appointment, you will be charged as a "Missed Appointment."

No Food or Beverages

No food or beverages are allowed to be consumed in our waiting room or clinical areas. This is to keep our office sanitary, clean, and up to ADA safety and allergy standards. We encourage you to have your morning brew before you arrive!

No Phones/Photography/Videos

Phone conversations, photography, recording, and videography is strictly prohibited for the sole purpose of protecting other patients’ privacy.  We encourage you to take phone calls outside our office. We understand that first dental visits can be exciting! If you would like a picture of your child, please inform the staff so we can make sure that we are in HIPAA compliance and no patient information is visible in the photo.
